SAB 125 Substance Abuse Case Management Lecture: 2 Lab: 2 Clinic: 0 Credits: 3 This course provides case management activities, including record keeping, recovery issues, community resources, and continuum of care. Emphasis is placed on establishing a systematic approach to monitor the treatment plan and maintain quality of life. Upon completion, students should be able to assist clients in the continuum of care as an ongoing recovery process and develop agency networking. This course is a unique concentration requirement of the Substance Abuse concentration in the Human Services Technology program.
Pre-requisite(s): SAB 120 . Course is typically offered in Fall. Student Learning Outcomes (SLOs) At the completion of the course, the students should be able to do the following:
- Differentiate between situations in which it is most appropriate for the client to self-refer to a resource, and instances requiring counselor referral.
- Describe how to access and arrange referrals to other professionals, agencies, community programs, or other appropriate resources to meet client needs.
- Explain in clear and specific language the necessity for, and process of, referral to increase the likelihood of client understanding and follow-through.
- Create accurate documentation of case management activities throughout the course of treatment.
- Recognize accepted principles of client record management.
- Explain how to protect client rights to privacy and confidentiality in the preparation and handling of records, especially in relation to the communication of client information with third parties.
